Subacute Care is the step between hospital and home. At Innova Health Systems we continue the patient’s Care Plan until the patient is at a sufficient level to return home.
Rehabilitation Services provide individuals with a comprehensive “Rehab” program to help them regain mobility skills and maximize their level of function.
Long Term Care is available to those residents in need of a friendly, caring residence that provides comprehensive healthcare services and assistance in daily living.

Innova is a recognized leader in quality subacute care. Whether recovering from an accident, illness or surgery, after a hospital stay, Innova Health & Rehab’s team of multi-disciplinary specialists work together to provide an individualized plan of care to address the patients’ needs. Our Subacute Units have an assigned Medical Director and are managed by the Director of Nursing. Each Unit is supported by a Subacute Nurse Manager, highly trained Nursing Staff, the Attending and Consulting Physicians, Social Workers and Therapeutic Recreation Staff.

Innova’s Subacute Care Teams organize the safe transition from hospital to home by continuing the Care Plan until patients achieve the highest level of independence and function. Our Teams provide comprehensive treatment and quality individualized care in a warm and caring, home-like environment.

Each Subacute Care Unit consists of 80 beds, including 60 Private Rooms.
Comprehensive medical services are available to each patient, with particular expertise in the following areas.

Innova’s Subacute Care Unit, has 70 beds including 50 Private Rooms. The medical services provided in these Units are varied, with particular expertise in the following areas:

Rehabilitation Services: Staff of 11 provides comprehensive physical and occupational therapy, with contract speech therapy, six days a week.
Respiratory Therapy: Our on-site Respiratory Therapist provides direct patient care five days a week, and on-call 24/7.
Wound Care: Innova provides a full-time Wound Care Nurse who directs the care of skin ulcers, diabetic wounds and post surgical wounds.
Infectious Disease Management: A Medical Director of Infectious Disease oversees the Antibiotic Therapy, Infection Control and Patient Isolation Programs.
Major Joint and Major Fracture: Orthopedists and Physiatrists have structured a program that includes “clinical pathways” for total knee and total hip, and major fracture rehabilitation.

Subacute Care at Innova Health & Rehab provides comprehensive treatment and quality individualized care in a home-like environment to assure the patient has a successful return to their home.

Innova Health & Rehab is committed to be the “market leader” in Post Acute Rehabilitation Services. Innova provides individuals with a comprehensive rehabilitation program designed to help patients regain skills in mobility and maximize their level of function, while recovering from a post acute illness or disabling injuries. Our fully equipped Rehab Gym is supported with 70 Subacute Beds, with 50 private rooms.

Rehab Services are provided six (6) days a week, Monday – Saturday.

Innova’s Rehab TeamOur professional staff includes 11 full-time, licensed, professional caring staff members:

A Director of Rehab
Four (4) Physical Therapists
Four (4) Occupational Therapists
Two (2) Rehab Aides
Speech Therapy is provided as a consult service
Consultant Physiatrists direct the Rehab Care and Chair the Care Planning and Discharge conferences.

The Rehab Team is supported by Attending Physicians, Nursing Staff, Respiratory Therapy, Social Work and Therapeutic Recreation Staff
