Merwick Care Center, a unit of Princeton HealthCare System, provides compassionate transitional care and long-term care services.
The Merwick Transitional Care Unit acts as a bridge between hospital and home, providing care for patients recovering from certain surgeries or prolonged illnesses.
The Merwick Long-Term Care Unit provides care for people who can no longer reside in their homes.
Merwick Care Center Transitional Care Unit
Merwick Care Center (MCC), a unit of Princeton HealthCare System, provides an interdisciplinary and personalized approach to caring. Merwick Care Center is a 76-bed skilled nursing facility that includes a Transitional Care Unit. This unit acts as a bridge between hospital and home by helping patients overcome the challenges of recovering from an injury, illness or surgery.
Merwick's Transitional Care Unit (TCU) is staffed by a skilled team of board certified physicians and health professionals. They provide caring and experienced treatment for patients who are recovering from hip or single knee replacements, surgeries and prolonged hospital stays that have resulted in debilitation.

Continuity of Care
If you or a loved one is a patient at University Medical Center at Princeton (UMCP), Merwick's physicians and staff have direct, computerized access to your hospital medical records. This helps to ensure a smooth transition from one level of care to the next. Whether you are coming to Merwick from UMCP or another hospital outside of Princeton HealthCare System, the physicians and staff at MCC are committed to communicating with your personal physician about your care and progress throughout your stay and upon discharge.
Active & Involved Medical Leadership
Our TCU Medical Director is a board certified physician specially trained in physical medicine and rehabilitation. In addition, patient care is overseen by our facility Medical Director who is board certified in geriatrics and internal medicine.

Merwick Care Center Long-Term Care Unit
The Merwick Care Center Long-Term Care Unit provides care and support for individuals who require assistance with their activities of daily living.
Our residents' care is overseen by a Medical Director who is a physician with board certification in both internal medicine and geriatrics. The exceptional quality of life and care provided by the Long-Term Care Unit has led to commendations from our residents, their families and physicians in the community.
Like University Medical Center at Princeton, Merwick Care Center is a unit of Princeton HealthCare System. This relationship ensures residents have 24-hour access to PHCS' continuum of care. If emergency care is ever necessary, UMCP is just minutes away.
We encourage resident and family involvement in all aspects of residential life and in making decisions about their care. Residents also determine the level of participation that is appropriate for them, developing their own schedules of activities and quiet times.
Amenities include lush grounds where residents can stroll or plant their own gardens, a sunny library and two relaxing solariums for spending time with family or friends.
